Legacy MD Membership Plan – Terms and Conditions

Membership Commitment Agreement

1. Minimum Commitment Period

By enrolling in the Legacy MD Membership Plan, you agree to a minimum 12-month commitment. This term begins on the date of your enrollment and automatically renews unless terminated as outlined below.

2. Early Cancellation Policy

You may cancel your Membership Plan at any time; however, early cancellation will result in the entire remaining balance for the 12-month term becoming immediately due, unless a written exception is granted by the Practice. Any such exception must be made in writing and signed by an authorized representative of Legacy MD.

3. Payment Terms

Membership fees may be paid monthly, quarterly, or annually as agreed upon at the time of enrollment. Failure to make timely payments may result in suspension or termination of membership benefits, and the full remaining balance for the 12-month term will be considered due.

4. Non-Transferable

Memberships are non-transferable and may not be shared or reassigned.

5. Amendments

Legacy MD reserves the right to amend these Terms and Conditions at any time. Any such changes will be communicated to members in writing.

6. No Refund Policy

Legacy MD does not offer refunds for any membership fees paid. All payments made toward the Membership Plan are final and non-refundable, regardless of usage or early cancellation. Please ensure that you understand and agree to the terms before enrolling.
